Membership Terms and Conditions
Membership benefits are valid for one full year from the date of purchase and must be used within that period. Any unused services will expire at the end of the term and cannot be carried over or transferred to another individual. This membership is not an insurance plan and cannot be combined with any other discounts or promotional offers. All benefits apply exclusively to services provided within this office and are subject to the doctor’s discretion. Smile transformation services are excluded from this membership. Discounted rates are based on the office’s usual and customary rates (UCR) for specific procedures, which may change at any time. Membership fees are non-refundable once services have been utilized. If you choose not to renew your membership at the end of the 12-month term, a $50 reactivation fee will apply in addition to the standard membership fee upon re-enrollment.
